At this time, I am not a participating provider on insurance panels. I can supply you with a “superbill” at your request, which you can submit to your insurance company for possible reimbursement. A superbill is an itemized form reflecting rendered services. Please note that not all insurance companies reimburse out-of-network providers, however, I am happy to verify your out-of-network insurance benefits and review them with you prior to beginning treatment. I have also subscribed to a platform called ‘reimbursify’ which will fill the out-of-network claims on your behalf.
